Client Service Agreement & Informed Consent

SM Greatrex Pty Ltd – Scott Greatrex

1. Nature of Services

Counselling and coaching services are provided by Scott Greatrex through SM Greatrex Pty Ltd.

Services are designed to support mental wellbeing, emotional regulation, behavioural change, conflict resolution, and decision-making clarity.

These services are not a substitute for medical, psychiatric, psychological, or emergency mental health care.

If you are experiencing a crisis or require urgent support, contact 000 or Lifeline (13 11 14).

2. Scope of Practice

Services are delivered within three core areas:

A. Men’s Mental Health & Identity
Support for stress, burnout, anger, identity erosion, high-functioning anxiety, performance pressure, and life transition.

B. Financial & Business-Related Distress (Non-Financial Advice)
Support for the emotional and behavioural impact of debt, restructuring stress, insolvency exposure, business pressure, and leadership burden.

This service does NOT provide financial advice, legal advice, credit advice, accounting advice, or insolvency services. Financial matters may be discussed for emotional processing and behavioural clarity only.

C. Conflict Resolution
Support with communication breakdown, boundary setting, partnership tension, and personal or workplace conflict.

If client needs fall outside this scope including acute mental health crisis, significant substance dependency, or complex psychiatric conditions referral to an appropriate professional may be required.

3. Confidentiality

All sessions are confidential and conducted in accordance with professional and ethical standards.

Confidentiality may be legally breached where:
• There is risk of serious harm to yourself or others
• Child abuse or neglect is disclosed (mandatory reporting)
• Records are subpoenaed by a court
• You provide written consent for information release

Online sessions are conducted using secure platforms; however, absolute digital security cannot be guaranteed.

4. Session Structure

Standard sessions are 60 minutes unless otherwise agreed.

Frequency is determined collaboratively.

5. Fees, Deposits & Payment

• A 50% deposit is required at the time of booking to secure your appointment.
• The remaining balance is payable prior to or at the commencement of the session.
• Fees are listed in Australian Dollars (AUD).
• Outstanding balances may result in cancellation or suspension of future appointments.

Medicare & Rebates:
Services provided by Scott Greatrex are not Medicare-registered services.
No Medicare rebate applies.
Private health rebates may not be applicable unless otherwise advised.

6. Cancellation Policy

• A minimum of 24 hours’ notice is required to cancel or reschedule without charge.
• Cancellations within 24 hours will result in forfeiture of the deposit and may incur the full session fee.
• Non-attendance (no-show) is charged in full.

In genuine emergencies, discretion may be applied.

7. Informed Consent

By booking, you acknowledge that:

  • You are voluntarily engaging in counselling/coaching services.

  • Sessions may involve emotional discomfort or confronting material.

  • Outcomes cannot be guaranteed.

  • You may withdraw from services at any time.

  • You may decline any suggested intervention.

8. Client Responsibilities

You agree to:

  • Attend sessions on time

  • Communicate honestly

  • Engage actively in the process

  • Provide appropriate notice if unable to attend

9. Limitation of Liability

SM Greatrex Pty Ltd and Scott Greatrex are not liable for:

  • Decisions made by you outside of sessions

  • Personal, business, or relational outcomes

  • Indirect or consequential loss

  • Technical disruptions beyond reasonable control

Nothing in this agreement excludes rights under Australian Consumer Law.

10. Termination

Either party may terminate services at any time.
Referral options may be provided where appropriate.

By proceeding with your booking, you confirm that you have read, understood, and agree to these terms.
